- Location: Tulsa, OK
- Start date: February 2025- Ongoing
- Schedule: Mon-Fri 8;30a-5;30p
- Patient volume: 25-35 patients/day
- Patient age: 100% pediatric
- Required procedures: extractions, space maintainers, fillings, crowns
QUALIFICATIONS:
- Active OK license
- Medicaid enrollment required (we can enroll the provider if needed)
