- Schedule: M 8a-5p, Th 8a-5p, F 8a-1p
- Patient Ages: 90% Adult/ 10% Peds
- Cases: Fillings, Crowns, Simple Extractions
- Support Staff: Support Staff: 2-3 Assistants, 2 Front Desk, 2-3 Hygienists.
- Credentialing timeframe: NA
