ASSIGNMENT DETAILS:

  • Locations: Fort Wayne, IN
  • Start date: ASAP-Ongoing
  • Schedule: Mon- Fri 8:45a-6p
  • Required procedures: General dentistry and restorative
  • Credentialing timeframe: 30 days (can be quicker if credentialed with Medicare)

QUALIFICATIONS AND REQUIREMENTS:

  • Active IN license
  • Certifications: CPR, BLS
